This service provides a range of home-based assessment and intervention services designed specifically for older adults or persons with disabilities.
Areas of specialty include:
- Falls prevention assessments and interventions
- Home modifications including bathrooms, ramps or access modifications.
- Equipment prescription including seating and wheelchair prescriptions.
- Community access.
- Assessment and equipment prescription to maximize activities of daily living for the individual.
